The NDIS Support Coordination team helps connect your funded supports with local service providers. A trained Support Coordinator will help you utilise your funding and choose providers that meet your needs.
If your plan includes Support Coordination, our Support Coordinators can help you manage your funded supports and providers to help you get the most out of your NDIS plan. Our Support Coordinators negotiate with providers on your behalf to ensure you get the help you need at a fair price.
Aftercare’s Support Coordinators will also help you prepare for your plan review and help you build the skills needed to exercise choice and control over your supports. Aftercare has Support Coordinators throughout Queensland, New South Wales and Victoria.
Understanding Levels of Support Coordination
Aftercare provides two levels of Support Coordination. These levels include:
- Support Connection: This service connects you with various supports, including informal, community and funded, to help you achieve your goals and live more independently. This support is often only short-term.
- Support Coordination: This service helps you develop a stronger understanding of your NDIS plan and how to best implement it. Aftercare’s Support Coordinators can work with you to optimise your plan and help you build your capacity for maintaining relationships and making decisions. Support Coordination is often a longer-term support service.
