Aftercare Programs
Aftercare Promotes Partnerships across all services
Since 1907 Aftercare has promoted partnerships with government agencies and the community to ensure that all people supported through its services have access to services that can create a greater independence in their community of choice.
Throughout the years Aftercare has worked in partnership with the NSW Department of Health and NSW Department of Housing to provide stability of accommodation and the support that people need to maintain that accommodation.
Other successful partnerships at both formal and informal levels are with TAFE, Education facilities, day programs and providers of social and leisure activities that can enhance Aftercare's existing services.
Health and Wellbeing
Aftercare actively promotes a healthy lifestyle by providing a range of social and recreational activities aimed at assisting the individual to experience and participate in a number of community based events.
Each individual supported by Aftercare has a right to access services to improve or assist their physical health needs. Where needed, Aftercare staff assist in making appointments to ensure that individual needs are met.
Continuous Improvement Program
Aftercare ensures that all services are planned, implemented and evaluated in keeping with the legislative requirements of the Disability Services Acts and Standards and the National Standards for Mental Health Services.
Families, friends and advocates
Aftercare recognises the value of families, friends and advocates and encourages and supports each individual to strengthen existing relationships or seek new ones if the individual wishes to do so.
